Another wish ...

I've just been reminded that some books have more than = one index.  For
example, such a book might have "Author = Index" plus "Subject Index".

Support for more-than-one index would be nice, = e.g.
   ... by the lake\index[subject]{Lake = Michigan} ...
but
   ... Laurence's\index[author]{D. H. = Laurence} novels ...
