Regarding typewriter-like output, I have a set of cmpica fonts (text only right now) which handle converting at the typeface level TeX input to typewriter output (e.g., `` and '' go to ", --- goes to --, -- to -, etc.) If there's sufficient interest, I suppose I could convert math as well, but that doesn't seem as important.
