(I doubt whether he anticipated that the result would run to 138 pages!) Frank now suggests that I should e-mail you all a copy. I'll start to = do that tomorrow. As far as I remember there's a root file, a .bbl file, and a number of files that are \include-d. In the hope of avoiding problems due to lack = of filestore at the receiving end, I'll mail a little every day for about 5 days. I'll mail the root file last. Thus, when you get the root file = you should have everything you need to typeset and print the document. You'll have seen much of the material before, but may find it convenient = to have it all consolidated in one place.
