I concluded that the composite font mechanism proposed by Don Knuth does indeed solve the problems in a general way, and I therefore conclude that mappings should NOT be in macro packages. [I think TeX should have originally supported a mapping array for each font; that mapping could have been set when a font was loaded. However, it is not obvious that mapping implemented at the macro level would be sufficiently efficient.]
